全麻与硬膜外麻醉用于妊娠高血压综合征剖宫产术临床观察

目的探讨全麻与硬膜外麻醉在妊娠高血压综合征（简称妊高征）急诊剖宫产术中的应用。方法42例妊高征患者随机分为全麻组和硬膜外组，每组21例，全麻组采用丙泊酚、司可林快速诱导插管后控制呼吸复合七氟醚吸入全麻，硬膜外组硬膜外穿刺成功后给予2％利多卡因进行硬膜外麻醉，记录两组患者诱导前10min（T1）、新生儿娩出即刻（T2）、术毕即刻（T3）的MAP和HR，记录两组患者的手术时间、失血量、输液量、硝酸甘油用量和术中麻黄素使用例数，同时记录新生儿Apgar评分〈7分例数。结果两组他和T3时MAP均显著下降（P〈0．01）；两组患者术中硝酸甘油用量无明显差异，而诱导至划皮时间、输液量和麻黄碱使用例数全麻组明显少于硬膜外组（P〈0．05或P〈0．01），两组新生儿Apgar评分差异无显著性（P〉0．05）。结论全麻和硬膜外麻醉用于妊高征剖宫产术都是可选择的安全麻醉方式。
