奈达铂联合化疗治疗恶性肿瘤的临床分析

目的观察奈达铂联合其他化疗药物治疗头颈部肿瘤、非小细胞肺癌、食管癌、妇科癌症的近期疗效和不良反应。方法入组共82例。奈达铂80～100mg/m2,加入500ml生理盐水中静脉滴注2h,每4周重复。结果头颈部肿瘤治疗组15例,CR4例(26·7%)PR7例(46·6%),有效率73·3%。非小细胞肺癌观察组25例,CR3例(12%),PR5例(20%),SD10例(40%),PD7例(28%),有效率RR为32%(8/25)。食管癌22例,CR2例(9%)、PR7例(31·8),RR40·9%。妇科癌症20例,结合放疗,全组病例有效率35%。不良反应主要有骨髓抑制,胃肠道反应少见,肝肾功能损害轻微。结论奈达铂对食管癌、头颈部肿瘤疗效优于顺铂。在非小细胞肺癌、妇科肿瘤疗效与顺铂相近,但是不良反应轻,主要为骨髓抑制,多数患者耐受良好。

Clinical analyse of nedaplatin in treating malignant tumor

Objective To observe the efficacy and the side effects of nedaplatin in treatment of head and neck squamovs cell carcinoma,non-small-cell lung cancer(NSCLC),esophageal cancer,gynecology cancer.Methods Eighty-two patiens were studied.Nedaplatin was administered at a dose of 80-100 mg/m2 with 500 ml of saline by slow drip infusion for 2 hours minutes,at intervals 4 weeks.Results Of 15 patients with head and neck squamovs cell carcinoma,4 patients were complete remission,7 patients were partial remission,the response rate was 73.3%.Of 25 patients with non-small-cell lung carcinoma,3 patients were complete remission,5 patients were partial remission,the response rate was 32%.Of 22 patients with esophageal cancer,2 patients were complete remission,7 patients were partial remission,the response rate was 40.9%.Of 20 patients with gynecology cancer,who combined with radiotherapy,the response rate was 35%.The dose-limiting toxicity was bone marrow depression,renal toxicity was not observed and gastrointestinal toxicity was mild.Conclusion The result of combined chemotherapy with Nedaplat is more effective in the treatment of esophageal cancer and neck squamovs cell carcinoma than cisplatin.It is similar to cisplatin in treating non-small cell lung cancer and gynecology cancer,but the side effect is mild,and its main side effect is bone marrow depression.
