不同喂养方式对婴儿血浆Ghrelin、Leptin水平及生长发育的影响

目的通过检测不同生长阶段的婴儿血浆饥饿素（Ghrelin）和瘦素（Leptin）的浓度以探讨不同喂养方式对于婴儿生产的影响。方法选取2012年1月到10月于浙江省三门县人民医院儿科门诊体检的80例非混合喂养的3月龄婴儿，按喂养方式分为两组：配方奶粉喂养组（FF组）48例，母乳喂养组（BF组）32例。两组均于3月龄时检测血浆Ghrelin、Leptin、胰岛素和空腹血糖水平；于3月龄和6月龄的时候分别记录喂养方式和生长发育相关指标。结果在3月龄时，BF组婴儿的体重、身长、三头肌皮肤褶皱厚度（TSF）以及血浆Ghrelin水平明显高于FF组（t值分别为2．073，5．162，3．419，2．319；均P〈0．05）。3月龄时，BF组的婴儿血浆Ghrelin与TSF呈明显正相关（r=0．719，P〈0．001），血浆Leptin水平与婴儿体重（r=0．512，P〈0．01）、TSF（r=0．632，P〈0．001）和体重增长量呈正相关（r=0．429，P〈0．05）。结论母乳喂养婴儿血浆中高水平的Ghrelin在婴儿生长发育的最初3个月发挥着重要作用。

Influence of different feeding patterns on serum Ghrelin,Leptin level and development of infants

Objective To explore the influenee of different feeding patterns on infant development by detecting the serum concentration of Ghrelin and Leptin at different stage. Methods Eighty 3-month-old non-mixed feeding infants, taking physical examination in outpatient pediatric clinic of Sanmen People＇ s Hospital from January to October 2012, were divided into 2 groups ： fornmla-fed （FF） group （ n = 48 ） and breast-fed （BF） group （n = 32）. Serum level of Ghrelin, Leptin, insulin and fasting blood glucose of two groups were detected at 3 month of age, and feeding patterns and development indexes of them were recorded at 3 and 6 month of age respectively. Results Body weight, length, triceps skin fold thickness （ TSF）, and serum Ghrelin level were significantly higher in BF group than in FF group （ t value was 2. 073, 5. 162, 3. 419 and 2. 319, respectively, all P 〈0. 05）. In BF group serum Ghrelin was correlated with TSF （ r = 0. 719, P 〈 0. 001 ）, and serum Leptin was correlated with weight （ r = 0.512, P 〈 0.01 ）, TSF （ r = 0. 632, P 〈 0.00l ） and weight gain （ r = 0. 429, P 〈 0.05 ） at 3 month of age. Conclusion Higher serum Ghrelin level in BF group might play a role in faster growth during the first three month of age.
