梅尼埃病患者内淋巴囊减压术后的生活质量研究

目的应用所设计的梅尼埃病治疗效果问卷(Meneiere’s disease oucomes questionnaire,MDOQ),对内淋巴囊减压术后梅尼埃病患者的生活质量(quality of life,QOL)进行评估研究。方法病例来自1982年1月至1998年1月药物治疗失败并进行了内淋巴囊减压术的68例梅尼埃病患者。MDOQ由三方面组成:身体的、情感的和社交的健康与愉快,有19项配对的问题(术前和术后),比较术前与术后QOL得分。结果术后QOL得分总的变化为27.8分(32~86)(P<0.001),QOL得分与MDOQ得分有很密切关系(P<0.05)。内淋巴囊减压术后QOL相应改善的MD患者占83.8%,无变化的患者占5.9%,变差的患者占10.3%。结论MDOQ是一新的对梅尼埃病患者QOL很好的评估方法,与治疗后梅尼埃病患者QOL的变化相一致。

A Study of Quality of Life of Patients with Meniere's Disease after Endolymphatic Sac Surgery

Objective The Meniere's disease outcomes questionnaire(MDOQ) was designed to measure the quality of life who recieved endolymphatic sac decompression for the treatment of Meniere's disease.Methods Studied were eighty-four patients with Meniere's disease who underwent endolymphatic sac decompression from Jan 1982 to Jan 1998, after failing a course of medical management. The Meniere's disease outcomes questionnaire was developed,consisting of questions in three domains to assess the quality of life: physical, emotional, and social well-being. The Meniere's disease outcomes questionnaire with 19 multiple-choice questions were paired for pre- and postoperative conditions, and one global quality-of-life question. Completed questionnaires were returned by 68 patients. The preoperative quality-of-life score was compared with the postoperative quality-of-life score. The main outcomes measure was the change in quality-of-life score.Results Overall, the mean change in quality-of-life score was 27.8 points (range, 32 to 86) (P<0.001). The change in Meniere's disease outcomes questionnaire quality-of-life score was highly correlated with that of the global question score (P<0.01).Quality of life was improved in 83.8% of respondents, unchanged in 5.9% of patients, and poorer in 10.3% of patients after endolymphatic sac decompression.Conclusion The Meniere's disease outcomes questionnaire is a new instrument to judge the quality-of-life and is a valid measure for patients with Meniere's disease, and is responsive to measuring change in quality of life after treatment.
