胸腔积液中CA19—9、CEA及ADA联合检测的诊断价值

目的探讨检测糖链抗原19—9（CA19—9）、癌胚抗原（CEA）及腺苷脱氢酶（ADA）对鉴别胸腔积液性质的临床价值。方法分别对40例恶性及良性胸腔积液患者中CA19—9、CEA、ADA含量水平检测，计算两组中三个指标的表达情况、单独及联合检测阳性率，以及对恶性胸腔积液的敏感性、特异性及诊断符合率。结果两组间三个指标含量差异均具有统计学意义（P〈0．05）；联合检测可以提高检测阳性率及诊断符合率，其中以CEA＋ADA最好，其敏感性、特异性及诊断符合率分别为97．5％、87．5％、92．5％。结论CA19—9、CEA在恶性胸腔积液中含量增高，ADA含量降低；CEA与ADA联合检测有助于提高恶性胸腔积液的检测价值。

Clinical values of detection of CA19 -9 CEA and ADA in pleural effusion

Objective To detect the expression of CAI9 -9,CEA and ADA in Pleural fluids, and investigate the diagnostic values between malignant and benign effusions. Methods Respectively checking the level of 40 examples of malignant and benign effusions of CA19 -9, CEA and ADA. The expression levels of CA19 -9, CEA and ADA in the two groups were calculated, and their diagnostic positive rates, sensitivities, specificities, diagnosis accordance rates were evaluated. Results Between the two groups, the level of the three indicators was significant difference（ P 〈 0. 05 ）. The combined assay of three tumor markers can increase the sensitivity and diagnostic accordance rates in diagnosing malignant pleural effusions, the diagnostic efficiency of CEA and ADA combined is best, the sensitivity, specificity and diagnostic coincidence rate were 97.5% ,87.5% ,92. 5%. Conclusion The content of CA19 -9, CEA in malignant pleural effusions is high, ADA is low. The combined assay of CEA and ADA can help to increase the diagnosing in malignant pleural effusions.
