13C尿素呼气试验、组织学检查和快速尿素酶试验对幽门螺杆菌感染的诊断价值比较

目的评价13C-尿素呼气试验（UBT）、组织学检查和快速尿素酶试验（RUT）在根除治疗前后诊断Hp感染的准确性。方法158例胃镜诊断的慢性胃炎患者，Hp阳性率72．15％。阳性患者于治疗前及停药后4周分别行胃镜及UBT检查。结果治疗前后不同方法诊断Hp感染的灵敏度UBT（92．11％、91．67％）及组织学检查（90．35％、79．17％）均高于RUT（75．44％、29．17％）。Hp感染程度治疗前分布不均匀，治疗后均明显下降。结论UBT在治疗前后灵敏度均最高；RUT在根除治疗后灵敏度低；多部位活检可提高诊断灵敏度。

Comparison of diagnosis value of 13 C-Urea breath test,histological examination and rapid Urease test for helicobactor pylori

Objective To evaluate the accuracy of 13C-Urea Breath Test（ UBT）, histological examination and Rapid U- rease Test （RUT） in diagnosing Helicobactor Pylori （Hp） infection before and after eradication therapy. Methods In a total of 158 patients with chronic gastritis diagnosed by gastroendoscopy, 72. 15% patients were Hp positive. Before therapy and 4 weeks after eradication therapy, patients with Hp positive were undergone UBT and gastroendoscopy for histological examination and RUT, respectively. Results The diagnosis sensitivity of UBT（92. 11% , 91.67% ） and histological method（90. 35% , 79. 17% ） all higher than RUT（75.44% , 29. 17% ） before and after therapy. The infectious degree of Hp was non-uniform distribution before therapy and decreased significantly after therapy. Conclusion The sensitivity of UBT was highest both before and after therapy,while the sensitivity of RUT was lower after therapy. Biopsy in multiple sites could improve the diagnosis sensitivity.
