血管回声跟踪技术在评估子痫前期孕妇颈动脉弹性的应用

摘    要:目的探讨血管回声跟踪技术在评估子痫前期孕妇颈动脉弹性的应用。方法选择32例轻度子痫前期和32例重度子痫前期孕妇为观察对象,同期选择32例健康的晚期妊娠做阳性对照,进行双侧颈总动脉血管内膜中膜厚度(IMT)和压力应变弹性系数(Ep)、硬度指数(p)、顺应性(AC)及脉搏波传导速度(PWVl3)等弹性指标检测,评价不同组别孕妇颈总动脉弹性指标的差异,并与IMT相关分析。结果轻度子痫前期组孕妇左右两侧IMT分别为(O.47±0.08)min和(0.46±0.09)mm,重度子痫前期组孕妇分别为(0.48±0.07)和(0.47±0.08)mm,各组间比较差异无统计学意义(P=0.764和P=0.803)。颈总动脉弹性参数轻度子痫组和重度子痫组患者左右两侧AC均显著低于对照组(P〈0.001)。与轻度子痫组比较,重度子痫组孕妇左右两侧AC显著降低(P=0.002和P=0.001)。同样的方法比较三组孕妇颈总动脉其他弹性参数,发现轻度子痫组和重度子痫组孕妇左右两侧β、Eβ和PWVβ水平均高于对照组,差异具有统计学意义(P〈0.001)。进一步做相关分析显示,IMT与AC呈负相关(r=-0.473),与IMT与β、EP、PWVβ呈正相关,相关系数分别为0.679、0.612、0.642。结论血管回声跟踪技术可以早期发现子痫前期孕妇颈总血管弹性变化。

Abstract:Objective To investigate the application value of echo tracking technique for assessing the elasticity of gener- al carotid artery in pregnant women with preeclampsia. Methods Totally 32 pregnant women with mild preeclampsia and 32 pregnant women with severe preeclampsia were collected as experimental group, and 32 normal pregnant women in the late trimester pregnancy formed a control group. We assessed intimal-media thickness(IMT) and the vascular e- lastic indexes of these participator, including elasticity modulus ( Ep), stiffness parameter ( 13), arterial compliance (AC) and one-point wave velocity ( PWVI3 ), and then their relationship was analyzed. Results The IMT of left and right carotid artery in mild preeelampsia group was ( 0. 47 ± 0.08 ) and ( 0. 46 ± 0. 09 ) m, was ( 0. 48 ± 0. 07 ) and (0. 47 ±0. 08)ram in severe preeclampsia group. There were not significant difference between two groups. In the com- paration of vascular elastic indexes, we found the AC was lower in mild and severe preeclampsia group, and patients with severe preeclampsia had much lower AC (left: P = 0. 002 and right: P = 0. 001 ). However, compared with control group, the 13,Ep and PWVI3 was higher in mild and severe preeclampsia group( P 〈 0. 001 ), and severe preeclampsia group had highest among three groups. Advanced analysis shows that IMT had significant negative correlation with AC ( r = -0. 473), and had significant positive correlation withβ, EP and PWVβ, The correlation coefficient were 0. 679, 0. 612 and 0. 642, respectively. Conclusion The echo tracing technique can find the early changes of elasticity of bi- lateral common carotid arteries in pre-eelamptie women.
