摘    要:目的 :探讨肝血管瘤在超声 ( US)、CT及 MRI的影像特点 ,比较不同检查方法对病灶的敏感性及特异性。方法 :经手术病理及血管造影证实的 4 0例肝血管瘤病人 ,均进行 US、CT及 MRI检查。对不同检查方法的影像表现及检出率进行比较 ,并对敏感性及特异性进行统计学处理。结果 :US、CT及MRI对肝血管瘤的敏感性分别为为 70 %、92 .5%和 95% ,特异性分别为 4 7.4 %、73.7%和 94 .7% ,不同方法之间的敏感性有显著差异 ( P<0 .0 0 5) ,MRI对肝血管瘤的敏感性高于 US及 CT。结论 :US可做为肝血管瘤的筛选方法 ,采用螺旋 CT或 /和 MRI更有利于定性 ,联合运用各种检查手段 ,对提高肝血管瘤的诊断准确性有很大价值

Abstract:Objective:This study was to inquire into the image characteristics of hepatic hemangioma shown by the ultrasound, CT and MRI and to compare the sensitivity and specificity of the foci by different methods. Methods:Forty patients with hepatic hemangioma(12 male and 28 female) were proved by pathological specimens removed from operation and by angiography and 25 experienced intervening treatment and 15 underwent operation.All the 40 patients received the examinations of ultrasound,CT and MRI.Then the image manifestations and detectable rates by different methods were compared ,and the sensitivity and specificity were statistically analyzed .Results:The sensitivity of ultrasound,CT and MRI to hepatic hemangioma was 70% , 92 5% , and 95% respectively; whereas, the specificity was 47 4% ,73 7% and 94 7% respectively .The sensitivity and specificity detected by different means were different ( p <0 005) . It was also found that the sensitivity and specificity of hepatic hemangioma by MRI were higher than those by ultrasound and CT. Conclusion:Ultrasound was the first choice of all the methods when hepatic hemangioma would be detected and at the same time,spiral CT scanning and MRI could be used to reconfirm the disease .It was necessary to combine different methods so as to raise the diagnostic level of hepatic hemangioma.
