人卵裂期胚胎玻璃化冻融体外培养时间对妊娠结局的影响

摘    要:目的研究人卵裂期胚胎玻璃化冻融体外培养时间对妊娠结局的影响。方法对我中心进行的解冻胚胎数不超过3枚,解冻胚胎全部存活并全部移植的274例患者分为当日移植组(A组)和次日移植组(B组),比较组间临床妊娠率、种植率和流产率。结果胚胎龄为第2天(D2)的共163个周期,其中A组130个周期,临床妊娠率、种植率和流产率分别为:23.08%、11.71%和6.67%,B组33个周期,临床妊娠率、种植率和流产率分别为:27.27%、11.84%和11.11%;胚胎龄为第3天(D3)的共111个周期,其中A组94个周期,临床妊娠率、种植率和流产率分别为:36.17%、19.35%和8.82%,B组17个周期,临床妊娠率、种植率和流产率分别为:35.29%、20.45%和16.67%。D2和D3胚胎当日移植组和次日移植组之间的临床妊娠率、种植率和流产率均无统计学差异。结论延长体外培养时间不显著影响玻璃化冻融卵裂期胚胎的发育潜能,可根据需要灵活安排玻璃化冷冻胚胎的解冻时间。

Impact of incubation time of vitrification-warming embryos on frozen-thawed embryo transfer outcomes

Abstract:Objective: To explore the relationship between incubation time of vitrification-warming embryos and the following frozen-thawed embryo transfer (FET) outcomes. Methods. A retrospective study was carried out for 274 FET cycles (163 cycles of day2 embryos and 111 cycles of day3 embryos) in which the embryos were transferred either on the same day (group A) or the next day (group B) after warmed. Clinical pregnancy rate, implantation rate and miscarriage rates were compared. Results: For day2 embryos, the clinical pregnancy, implantation and miscarriage rates were 23.08%, 11.71% and 6.67% in group A (130 cycles) and 27.27%, 11.84% and 11.11% in group B (33 cycles), respectively. For day3 embryos, the clinical pregnancy, implantation and miscarriage rates were 36.17%, 19.35% and 8.82% in group A (94 cycles) and 35.29%, 20.45% and 16.67% in group B (17 cycles), respectively. There was no difference between group A and group ]3 in the clinical pregnancy, implantation and miscarriage rates for either day2 or day3 cleavage-stage embryos.Conclusions. Prolonging the incubation time does not influence the development potential of vitrified/ warmed embryos.
