髂腹股沟区血供的应用解剖研究及其临床意义

目的 为临床合理应用髂腹股沟区骨瓣和皮瓣移位或移植术提供解剖学依据。方法 在31侧经动脉内灌注红色乳胶的成人标本上，解剖观测髂腹股沟区营养髂嵴前部骨质和腹部、会阴部皮肤血管的起源、走行、分支、分布、外径和吻合情况。结果 该区存在旋髂浅动脉、旋髂深动脉、旋股外侧动脉升支和臀上动脉深上支营养髂嵴前部骨质；腹壁下动脉、腹壁浅动脉、阴部外浅动脉和闭孔动脉营养腹部和会阴部皮肤。相邻血管之间互相吻合，彼此互补。结论 临床可根据不同需要选择带血管蒂的骨瓣、皮瓣行移位或移植术，并利用其互补性选择最佳术式。

Anatomic Study and Clinical Significance of Vascularized Bone Flap or Skin Flap in Ilioinguinal Region

Objective To provide anatomical basis for the transposition or transplanting of the vascularized bone flap or skin flap in ilioinguinal region.Methods The origin, course, branch, ditribution, external diameter and anstomosis of non-trunk vessels and nerves in the ilioinguinal region were observed in 31 adult corpses perfused with red latex through artery. Results There were superficial circumflex iliac artery, deep circumflex iliac artery, ascending branch of lateral circumflex femoral artery, deeply superior branch of superior gluteal artery in this region, which could form bone flap of anterior iliac crest. Inferior epigastric artery, superficial epigastric artery, lateral pudendal artery and obturator artery could form epigastric and pudendal skin flap.Each vessel had its characteristics, and could complement each other.Conclusion We should consider the different needs to choose the proper bone or skin flap for transposition or transplanting, and make use of their complementation to select the optimum form of operation.
