胬肉逆行切除并转移结膜瓣修复术

目的探讨显微镜下逆行法胬肉切除并转移结膜瓣修复的可行性及手术效果。方法79例(83眼)胬肉患者接受显微镜下手术后,随访6个月;沿胬肉上方切开结膜,用月形隧道刀切除结膜下变性组织;剥离方向是由胬肉尾部向头部,切除4～5mm长的结膜及胬肉处的结膜下变性组织;将巩膜表面血管热凝后,取角膜缘上一结膜瓣移植于暴露的巩膜上。结果全部术眼均顺利完成胬肉切除及转移结膜瓣手术,术后结膜瓣完全成活,无不良反应及并发症,随访期间胬肉无复发。结论逆行法胬肉切除并转移结膜瓣修复术治疗翼状胬肉效果良好,方法简便易行,具有较大的实用价值。

Retrograde pterygium resection with transplanting conjunctiva flap to repair pterygium

Objective To probe into efficacy and feasibility of retrograde ptergium resection with transplanting conjunctiva flap for repairing ptergium.Methods The83eyes of79patients with pterygium were underwent microscopic surgery and were followed up for6months.Conjunctiva incision was performed up the pterygium,crescent knife was used to remove degenerated subconjunctial tissues.Direction of dissection is from the tail of pterygium to the head.After dissection,4-5mm conjunctiva and degenerated subconjunctial tissues at pterygium place were cut down.Following hot coagulation of the surface vessel of scleral,a conjunctiva flap up limbus of cornea was removed and transplanted on the exposured scleral.Results The retrograde pterygium resection with translocating conjunctiva flap for repairing it were successfully performed on all eyes(83);transplanted conjunctiva flaps all were survival,Follow-ups show,there were no side reaction,complications,and ptergium reccurence.Conclusion The retrograde pterygium surgery with autograft tissue transplantation is effective and convenience,and has more practical value for treating ptergium.
