腹腔游离癌细胞测定在大肠癌治疗中的临床意义

目的：评价腹腔游离癌细胞测定的临床价值。方法：126例进展期大肠癌患者，术中探查前先用100ml生理盐水冲洗腹腔，然后从Douglas窝中取出50ml生理盐水进行细胞离心检测，共检测出癌细胞阳性72例（57.14%)，将其随机分成两组，一组接受早期腹腔温热化疗及静脉化疗，另一组接受静脉化疗，结果：静脉化疗组腹腔内复发9例，肝转移4例；早期腹腔温热化疗组，腹腔内复发2例，肝转移3例，两组复发率有显著差异；两组均未见严重并发症。结论：腹腔游离癌细胞测定有助于指导大肠癌术后早期进行腹腔温热化疗并对判断大肠癌预后有一定的价值。

Clinical significance of determining free cancer cells in abdominal cavity in the treatment of colorectal cancer

Objective: To evaluate the clinical value in measuring the free cancer cells in abdominal cavity. Methods: The abdominal cavities of 126 patients with advanced colorectal cancer were flushed with 100 ml physiological saline before the operative exploration. Then 50ml were drained from Douglas cavity for centrifugalizing. The positive free cancer cells were found in 72 patients (57.14%) and they were randomly divided into group A and group B. Group A received early intraperitoneal hyperthermia chemoperfusion and venous chemotherapy; group B only received venous chemotherapy. Results: Nine patients had recurrence in abdominal cavities and 4 had liver metastasis in group B; two patients had recurrence in abdominal cavities and 3 had liver metastasis in group A. There was significant difference between the recurrence rates in the two groups. No serious complication was found. Conclusion: The measurement of the free cancer cells in abdominal cavity is helpful to guide the early intraperitoneal hyperthermia chemoperfusion in patients with colorectal cancer after their surgical operations, and it also has some values in judging the prognosis of colorectal cancer.
