一期开放植骨在感染性骨折不愈合中的临床研究

摘    要:目的对比探讨一期与延期开放植骨在感染性骨折不愈合中的可行性。方法将我院2009年2月-2011年2月确诊收治住院的68例感染性骨折不愈合患者按入院的先后顺序分为两组,其中实验组38例行一期开放植骨,对照组30例行延期开放植骨,对比两组患者的临床指标。结果实验组肉芽组织完全覆盖时间、瘢痕愈合时间、骨折愈合时间、去除外固定架时问分别为(6.8±1.2)周、(9.7±1.7)周、(7.6±0.8)月、(9.7±1.6)月;对照组分别为(8.3±2.6)周、f11.5±2.9)周、(9.1±1.3)月、(11.2±2.1)月;实验组均优于对照组,差异有显著性(P〈0.05)。结论运用一期开放植骨治疗感染性骨折不愈合,可促进患者肉芽组织覆盖及瘢痕愈合,有利于加快骨折恢复,提高患者的生活质量。

Abstract:Objective To compare and explore the feasibility of one stage and delayed open bone grafting in the infected fracture nonunion. Methods 68 patients with infected fracture nonunion from Feb 2009 to Feb 2011 were collected, and according to admission number in hospital, they were randomly divided into two groups: 38 cases in the experimental group using one stage open bone grafting, 30 cases in the control group using delayed open bone grafting, then we compared the clinical indicators of two groups. Results The granulation tissue covering the bone grafting time, scar healing time, fracture healing time, removal of external fixation time in experimental group were (6.8 ± 1.2) weeks, (9.7 ± 1.7) weeks, (7.6 ± 0.8) months and (9.7 ± 1.6) months respectiveb; those in the control group were (8.3 ± 2.6)) weeks, (11. 5± 2.9) weeks, (9.1 ± 1.3) months, and (11.2 ± 2.1) months respectively, the difference of two groups was statistically significant (P 〈 0.05 ). Conclusion The use of one stage open bone grafting in the treatment for irffected fracture nonunion, could promote granulation tissue covering and wound healing of the patients, and accelerate fracture recovery time, and improve the quality of life of patients.
