应变率成像对急性前壁心肌梗死患者左心功能的评价

摘    要:目的:观察急性前壁心肌梗死患者心肌应变率(SR)的变化特点,探讨应变率成像(SRI)技术定量评价急性心肌梗死(AMI)患者的左室局部心肌功的临床应用价值。方法:应用SRI对20例AMI患者和30例正常人左室各室壁节段纵向收缩期、舒张早期及房缩期的峰值应变率进行测定,并以冠脉造影结果为标准进行对比分析。结果:SRS:AMI组基底部、中部水平所有室壁较正常对照组显著性减低,心尖水平除下壁外其他室壁较对照组显著性减低;SRE:AMI组不同水平左室所有室壁较对照组显著性减低;SRA:基底部前间隔、后壁、前壁、下壁较对照组显著性减低,中部后间隔、后壁、前壁较对照组显著性减低,心尖水平后间隔、前壁、下壁AMI组较对照组显著性减低(P<0.01和0.05)。结论:SRI技术是临床无创、定量评价急性心肌梗死局部心肌功能的有效方法。

Evaluation of left ventricular function in patients with acute left ventricular anterior myocardial infarction by use of strain rate imaging

Abstract:Objective:To observe the myocardial strain rate characteristic in patients with acute anterior wall myocardial infarction and evaluate the clinical value of strain rate imaging(SRI) in quantitative assessment of left ventricular(LV)region myocardial function.Methods: Twenty patients with acute myocardial infarction(AMI) and thirty control subjects were enrolled in the study.Longitudinal peak strain rate of each LV walls during systole,early diastole and atrium contraction(SRS,SRE,SRA) were measured at different levels(base,middle and apical).SRI data were compared with coronary angiography results.Results:SRS :all LV walls(base,middle level) and those of the other walls except for inferior wall(IW)of apical level were significantly decreased in AMI group(P< 0.05,P<0.01 respectively);SRE :all levels of LV wall in AMI group were significantly lower than those of the control group(P< 0.05,P<0.01 respectively);SRA : basal level of anterior interventricular septum(AS),posterior wall(PW),anterior wall(AW),IW,middle level of post septal(PS),PW,AW and apex level of PS,AW,IW in AMI group were also significantly decreased compared with control group(P< 0.05,P<0.01,respectively).Conclusion:SRI is atraumatic and quantative imaging diagnosing tool for the patients with AMI.
