应用耳区皮瓣和颞浅筋膜神经岛状瓣的一期耳再造

目的：报告一种应用耳区皮瓣和颞浅筋膜神经岛状瓣的一期耳再造方法。方法：应用耳区皮瓣和颞浅筋膜神经岛状瓣一期再造耳41例。支架分别用自体肋软骨，硅橡胶和多孔取乙烯（MEDPOR）材料。结果：29例软骨支架和3例MEDPOR支架无任何即时并发症，3例软骨支架再造耳有所变形。2例硅橡胶支架在术后1-3个月内外露，经3个月-11年的随访，再造耳形态满意，色泽良好并能恢复部分感觉功能。结论：联合应用颞浅筋膜神经岛状瓣与耳区皮肤为耳再造提供了良好的覆盖组织。

One Stage Ear Reconstruction by Using the Auricular Skin Flap and a Neurovascular Island Flap of Superficial Temporal Fascia

Objective: To report a technique of one stage ear reconstruction by using the auricular skin flap and a neurovascular island flap of superficial temporal fascia. Methods: 41 ears were reconstructed by using the auricular skin flap and a superficial temporal fascia flap in one stage. The material of auricular framework include autogenous rib cartilage, silicone and porous polyethylene(MEDPOR). Results: No any complication had been encountered in 29 reconstructed ears with cartilage framework and 3 ears with MEDPOR framework. Two silastic frameworks were extruded one to three months after surgery. 3 reconstructed ears with cartilage framework have some deformity. Under 3 months to 11 years follow-up, the profile of the reconstructed ear become much more delicate, and with pleasant color match. Adequate sensation could be recovered. Conclusion: The application of neurovascular island flap of superficial temporal fascia and auricular skin supply the satisfied cover tissue for auricular reconstruction.
