儿童再发性腹痛与上消化道疾病及幽门螺杆菌感染的关系

目的：探讨儿童再发性腹痛与上消化道疾病、幽门螺杆菌感染的关系。方法：对198例再发性腹痛(RAP)息儿进行胃镜检查及幽门螺杆菌检测。结果：组织病理学证实169例(85．4％)粘膜有炎症改变。慢性胃炎140例(82．8％)，食道炎15例(8．87％)，十二指肠球炎8例(4．73％)，消化性溃疡6例(3．55％)。组织学染色及快速尿素酶实验证实HP感染为97例(49．0％)。HP阳性者粘膜均有炎症改变。胃炎HP感染率为64．3％。而HP阴性胃炎者仅35．7％，相反，食道炎和十二指肠球炎者HP感染率低。消化性溃疡者其中5例(83．3％)HP阳性。1例(16．7％)HP阴性。结论：小儿LRAP中上消化道疾病占较高比率。HP感染可能是引起RAP的主要原因之一。

The Relation of Recurrent Abdominal Pain with Upper Gastrointestinal Inflammtion and Helicobacter Pylori

Objective To investigate the relationof recurrent abdominal pain with upper gastrointestinal inflammation and helicobacter pylori.Methods 198 children complaining of recurrent abdominal pain(RAP)underwent upper gastrointestinal endoscopy in order to identify any underling organic pathology and determine the prevalence of Helicobacter pylori(HP).Results Histologically confirmed mucosal inflammation was found in 169(85.4%),including chronic gastritis of 140 cases (82.8%),oesophagitis of 15 cases(8.87%),duodenitis of 8 cases(4.73%),peptic ulcer of 6 cases(3.55%).By histology and rapid urea test,97 cases(49.0%)were infected HP.Histologically confirmed gastritis was the most prominent finding in Hp positive children compared with HP negative(64.3% VS 35.7%).Conversely,oesophagitis and duodenitis were more common in HP negative children. Five of six cases of peptic ulcer were infected by HP.The incidence of peptic ulcer was higher in HP infected patients than in the HP negative group(83.3% VS 16.7% ).Conclusion Our data suggest that upper gastrointestinal pathology is more common than previously thought in children with RAP,while Hp infection is the main reason to cause RAP.
