摘    要:【目的】通过动物实验,研究不同愈合时间微螺钉种植体-骨界面颈部压力侧与非压力侧的组织学变化,探讨愈合期对颈部界面结合方式的影响。【方法】成年雄性杂种犬3只,每只犬每侧上颌颊侧骨板植入4枚种植钉,共24枚,分别进行愈合0、4、8周后加载200 g牵引力,加载8周时处死动物,取材,标记压力侧和非压力侧,两侧均制备扫描电镜标本、脱钙的HE染色标本及不脱钙的硬组织标本,后者行甲苯胺蓝及亚甲基蓝-碱性品红染色,光镜及扫描电镜下观察界面组织变化。【结果】即刻加力组颈部四周被纤维包绕,压力侧骨改建不如非压力侧活跃;愈合4周组压力侧骨改建较非压力侧活跃;愈合8周组压力侧与非压力侧骨改建较活跃,两侧差异不显著。【结论】随着愈合期的延长,无论压力侧还是非压力侧都逐渐由纤维愈合向骨性愈合转化。但即刻加力影响骨改建。

Abstract:【Objective】To observe the histological alterations of the bone-implant cervical interface between pressure and non-pressure sides under different heeling time and discuss the healing mode of jaw after insertion of microscrew implant.【Methods】 24 titanium miniscrews were inserted in the maxillary alveolar bone between the dental roots in 3 crossbred dogs.Healing after 0,4,8 weeks,200 g force were loaded to the implans.The dogs were sacrificed after 8 weeks.The samples including the microscrew implant and its surrounding bone tissue were extracted.The slices of MIA on the pressure and non-pressure sides were labeled.Scanning electron microscopy specimens and the decalcified HE stained specimens were prepared for both sides.At the same time,the undecalcified bone tissue specimens were stained by toluidine blue and methylene blue-basic fuchsin for both sides.These specimens were observed under optical microscope and scanning electron microscope.【Results】In the immediate loading group,the bone-implant enclosed by fibrous tissue in cervical,bone remodeling on the pressure side were inactive.While in the group after 4 healing-weeks,bone remodeling on the pressure side were more active than those on the non-pressure side.There was no significant difference between the 4 healing-weeks group and the 8 healing-weeks group.【Conclusion】With the prolongation of healing time,neither the pressure sides nor the non-pressure sides,fibrous tissues were replaced by neoformative bone gradually.But immediate loading influenced bone remodeling.
