人体肥胖测量参数对糖代谢异常风险的预测价值研究

摘    要:目的评价人体肥胖测量参数对糖代谢异常风险的预测价值。方法采用分层抽样方法对山东省4个城市20岁以上的3 011名常住居民进行流行病学调查,采用受试者工作特征曲线(ROC curve)分别比较不同性别体质量指数(BMI)、腰围(WC)、腰围臀围比值(WHR)、腰围身高比值(WHtR)对糖代谢异常的预测价值。结果①人群各指标预测糖代谢异常风险的最佳切点:男性BMI是24.95,WC是91.5 cm,WHR是0.905,WHtR是0.535;女性BMI是24.25,WC是84.5 cm,WHR是0.855,WHtR是0.525;②BMI、WC、WHR及WHtR的ROC曲线下面积:男性分别是0.588、0.614、0.588和0.627;女性分别是0.616、0.684、0.684和0.693,均以WHtR的曲线下面积最大。结论无论男女,WHtR是糖代谢异常的最佳预测指标,最佳切点男性是0.535,女性是0.525。

Abstract:Objective To investigate the relationship between anthropometric parameters and glucose metabolism disorder risk of the adults in Shandong province.Methods 3 011 residents aged over 20 years from 4 cities in Shandong province were investigated by cross-sectional and cluster sampling methods.The predictive values on glucose metabolism disorder risk of body mass index(BMI),waist circumference(WC),waist-to-hip ratio(WHR),and waist-to-height ratio(WHtR) were assessed by receiver operating characteristic curve(ROC) analysis and calculating the area under the ROC(AUC).Data of the subjects were analyzed by sex.Results ① The optimal cutoffs to predict glucose metabolism disorder risk were found: BMI 24.95 for men and 24.25 for women,WC 91.5 cm for men and 84.5 cm for women,WHR 0.905 for men and 0.855 for women,and WHtR 0.535 for men and 0.525 for women.② WHtR was found to have the largest areas under the ROC curve,respectively 0.627 for males and 0.693 for females.Conclusion WHtR is the best predictive indicator for the glucose metabolism disorder risk.The optimal cutoffs of WHtR to predict glucose metabolism disorder risk were found: 0.535 for men and 0.525 for women.
