体外药敏试验指导下的晚期肺癌并胸水的胸膜腔内化疗

目的　探讨MTT体外药敏试验指导晚期肺癌并胸水的胸腔化疗的价值。方法　 138例晚期肺癌并胸水的病人中 69例在胸腔化疗前接受了MTT体外药敏试验。其中MTT敏感组 58例 ,根据药敏结果指导胸腔化疗 ;MTT耐药组 11例和对照组 69例则凭经验胸腔化疗。结果　MTT敏感组有效率为 77.6% ( 4 5/58) ,MTT耐药组和对照组分别为 0 ( 0 / 11)及 4 4 .9% ( 31/ 69) ,差异有显著意义。敏感组与对照组在不同治疗阶段、化疗药物的小组比较中有显著性差异。本研究体外药敏和体内疗效的总符合率为 81.2 % [( 4 5+11) /69]。结论　MTT体外药敏试验指导肺癌并胸水的胸腔化疗是可行的

Intrapleural Chemotherapy for Advanced Lung Cancer Complicated by Pleural Effusion under the Guidance of Drug Sensitivity Assay

Objective:To probe into the value of MTT assay in vitro that guides intrapleural chemotherapy for advanced lung cancer complicated by pleural effusion. Methods:Of the 138 patients with advanced lung cancer complicated by pleural effusion,69 patients had been given MTT assay before intrapleural chemotherapy.58 patients in MTT sensitive group were given this intrapleural chemotherapy according to the results of drug sensitivity assay.The other 11 patients in MTT resistant group and 69 patients in the control group were given intropleural chemotherapy according to clinical experience. Results:The valid rate in MTT sensitive group was 77.6%(45/48).The difference between resistant group with valid rate 0(0/11) and control group with valid rate 44.9%(31/69) was significant.MTT sensitive group and control group are significantly different at different therapentic phases and in the comparison of subgroups with different drugs.The overall coincident rate of drug sensitivity in vitro and curative effect in vivo was 81.2%[(45+11)/69].Conclusion:The MTT assay in vitro was helpful in the intrapleural chemotherapy advanced lung cancer complicated by pleural effusion.
