连续64例胰十二指肠切除无胰漏并发症的临床经验

目的探讨预防胰十二指肠切除术后并发症的临床经验。方法回顾性总结胰十二指肠切除术64例的临床资料。结果全组无胰漏发生,术后并发伤口感染3例,胃排空障碍1例,肺部感染1例,胃肠出血1例,腹腔脓肿1例,胆瘘1例。并发症发生率为12.5%(8/64)。结论胰管内支撑引流、精细的胰肠吻合和有效的围手术期治疗是预防Whipple术后胰漏的关键。

Experience in no complication of pancreatic leakage for 64 cases pancreaticoduodenectomy

Objective To study the prevention of pancreatic leakage after pancreaticoduod-enectomy.Methods From January 1997 to June 2002,64 cases received pancreaticoduodenectomy were analyzed retrospectively.Results The postoperative complicated rate was 12.5%(8/64),including wound infection in 3 cases,residual gastric emptying dysfuntion in 1 case,pulmonary infection in 1 case,biliary fistula in 1 case,abdominal abscess in 1 case,gastrointestinal bleeding in 1 case.No postoperative pancreatic leakage was occurred and no hospitalization death was found in all cases.Conclusion Internal stent drainage of pancreatic duct,delicate pancreaticojejunal anastomosis and effective perioperative management are keys to prevent pancreatic leakage after pancreaticoduodenectomy.
