玻璃体切除联合晶体超声碎核术治疗晶体半脱位合并葡萄膜炎

目的：探讨玻璃体切除联合晶体超声碎核术治疗晶体半脱位合并葡萄膜炎的临床治疗效果。方法：对我院1998年6月至2001年12月期间共收治的9例(11眼)晶体半脱位合并葡萄膜炎的患者行玻璃体切除联合晶体超声碎核术，其中7例(9眼)未I期植入人工晶体，另2例(2眼)植入缝线型人工晶体，术后随访3个月到24个月，观察术后视力及并发症等。结果：术后3月所有患者最佳矫正视力均有不同程度的提高，其中≥0．6的6眼，0．3～0．6的3眼，另2眼最佳矫正视力0．1～0.3；术后并发症有手术后一周内前房的纤维索性渗出4例(5眼)，经药物治疗后很快吸收；术后复发性葡萄膜炎2例(2眼)均为植入缝线型人工晶体眼，药物治疗效果不佳。结论：玻璃体切除联合晶体超声碎核术是治疗晶体半脱位合并葡萄膜炎最为安全和有效的治疗方法，术后并发症少，视力恢复满意，但I期植入缝线型人工晶体可能导致葡萄膜炎复发。

Phacofragmentation and pars plana vitrectomy in patients with lens dislocation and uveitis

To investigate the effects of phacofragmentation and pars plana vitrectomy on lens dislocation and uveitis. Methods:11 eyes of 9 patients with lens dislocation and uveitis who underwent phacofragmentation and pars plana vitrectomy were analyzed,in which 2 eyes underwent ciliary sulcus fixation of intraocular lenses.The mean follow-up period was 9 months (3 to 24 months). Results:The best-corrected visual acuity improved in all eyes.In 6 of the 11 eyes the postoperative vision was 0 6 or better.The main postoperative complication included cellulose exudation in 4 eyes and the reactivity of the inflammatory in 2 eyes. Conclusions:The combined operation might be an effective and safe surgical method for lens dislocation and uveitis.
