副鼻窦恶性肿瘤的影像学诊断

目的:探讨副鼻窦恶性肿瘤的影像学表现,提高影像诊断水平。方法:回顾性分析我院诊断的副鼻窦恶性肿瘤42例。均行X线和CT检查,加冠状位CT扫描24例,行MRI检查7例,均经手术病理证实。结果:上颌窦源性22例,筛窦8例,蝶窦2例,另10例因肿瘤较大、侵犯范围广而无法判断来源。鳞癌28例,腺癌9例,纤维肉瘤4例,脂肪肉瘤1例。结论:X线对副鼻窦病变的诊断具有筛查作用,CT是明确诊断副鼻窦恶性肿瘤的主要方法,MRI对副鼻窦恶性肿瘤诊断与鉴别诊断具有重要意义。

The imaging diagnosis of the malignant tumors of paranasal sinus

Objective: To evaluate the imaging findings of malignant tumors of paranasal sinus, and to improve the imaging diagnostic level. Methods: Forty-two cases of the malignant tumors of paranasal sinus were retrospective analyzed. X-ray and CT were performed in all cases; coronal CT scan was performed in 24 cases; MRI was performed in 7 cases. All cases were confirmed by operation and pathology. Results: The malignant tumors were showed to originate from maxillary sinus in 22 cases, from ethmoidal sinus in 8 cases, from sphenoidal sinus in 2 cases. The origin of 10 cases could not be distinguished because of the size of the tumors with extensive aggression. Pathology showed squamous cell carcinoma in 28 cases, adenocarcinoma in 9 cases, fibrosarcoma in 4 cases, and liposarcoma in 1 case. Conclusion: X-ray plays a role in screening of paranasal sinus tumors. CT is a primary method to accurately diagnose the malignant tumors of paranasal sinus. MRI has significance in diagnosis and differential diagnosis of malignant tumors of paranasal sinus.
