缺血性脑卒中患者CRP变化和临床意义研究

摘    要:目的探讨急性脑卒中与炎性反应的关系以及C-反应蛋白(CRP)水平与脑梗死严重程度的相关性。方法急性缺血性脑卒中患者42例,根据第四届全国脑血管病会议制定的脑卒中患者临床神经功能缺损程度评分标准,按病情轻重分型。健康体检者30例为对照组,检测CRP。结果缺血性脑卒中组CRP(7.3±3.7)mg/L,明显高于对照组的(4.3±2.8)mg/L(P<0.01);轻型脑卒中组CRP(4.7±2.0)mg/L,与对照组比较,差异无统计学意义(P>0.05);中型脑卒中组CRP(6.8±3.0)mg/L,明显高于轻型脑卒中组(P<0.01)。轻型脑卒中组与重型脑卒中组CRP比较,差异有统计学意义(P<0.01)。中型脑卒中组与重型脑卒中组CRP比较,差异有统计学意义(P<0.01)。结论炎症与急性脑卒中的发生具有明确的相关性,炎性反应是急性脑卒中发病的一个重要危险因素,CRP水平与脑梗死的严重程度密切相关。

The changes and clinical significance of C-reactive protein levels in patients with cerebral ischemic stroke

Abstract:Objective To investigate the relationship between acute cerebral apoplexy and inflammatory reaction, and to explore the correlation between the levels of C-reactive protein (CRP) and severe degree of cerebral infarction. Methods 42 patients [ mean age(63.19 ± 9.09) yr,27 males and 15 females] with acute cerebral ischemic stroke were divided into light degree group (0 - 15 score) ,middle degree group (16 -30 score) and severe degree group ( 31 - 45 score ) acaording to the scoring criterion of neurologic impairment degree established by The Fourth National Cerebrovascular Disease Conference. The 30 healthy subjects [ mean age (56.97 ± 9.51 ) yr, 17 males and 13 females ] were used as control group, and the serum levels of CRP were detected for all the patients and controls. Results The levels of CRP [ ( 7. 3 ± 3. 7 ) mg/L ] in ischemic stroke group were significantly higher than those[ (4.3±2.8) mg/L] in control group ( P 〈0.01 ) ;the levels of CRP in light degree group[ (4.7±2.0)mg,/L] and control group had no signifi cant difference ( P 〉 0.05);the levels of CRP in middle degree group (6.8±3.0mg/L) were significantly higher than those in light degree group ( P 〈0.01 ). There were significant differences in the levels of CRP between light degree group and severe degree group ( P 〈 0.01 ) ,and there were significant differences in the levels of CRP between middle degree group and severe degree group ( P 〈 0.01 ). Conclusion The inflammation is closely correlated with the occurrence of acute cerebral apoplexy, and inflammatory reaction is an important risk factor in the pathogenesis of acute cerebral apoplexy, and the levels of CRP are closely related with the severe degree of cerebral infarction.
