乌司他丁联合连续性血液净化治疗急性呼吸窘迫综合征

目的]观察乌司他丁联合连续性血液净化治疗急性呼吸窘迫综合征的疗效.方法]将44例患者随机分为治疗组与对照组,每组22例.治疗组采用乌司他丁联合连续性血液净化治疗,对照组采用常规治疗.观察统计使用呼吸机天数、住ICU 时间及ICU病死率;并记录治疗前后患者呼吸频率、氧合指数、血氧饱和度等指标,监测患者临床症状、体征变化、胸部X线片、三大常规和生化检查.结果]治疗组其呼吸频率、氧合指数、血氧饱和度等指标的改善情况均优于对照组,且两组相比差异有显著性( P <0.05).治疗组呼吸机使用天数、入住ICU时间较对照组明显减少,病死率显著降低( P <0.01).结论]乌司他丁联合连续性血液净化治疗急性呼吸窘迫综合征可以提高呼吸窘迫综合征抢救成功率,降低病死率.

Clinical Study of Ulinastatin Combined with Continuous Blood Purification for the Treatment of Acute Respiratory Distress Syndrome

Objective] To observe the efficacy of ulinastatin combined with continuous blood purification on acute respira- tory distress syndrome（ARDS）. Methods] All 22 cases were divided into treatment group and control group. Treatment group was treated by ulinastatin combined with continuous blood purification and control group was treated by routine therapy. The＇day of mechanical ventilation, stay time and the mortality in ICU were observed. Respiratory frequency, oxygenation in- dex and blood oxygen saturation were recorded before and after treatment. Clinical symptoms and signs were monitored. Chest X-film and routine examinations were performed. Resuitsl Respiratory frequency, oxygenation index and blood oxygen satu- ration in treatment group were better than those in control group, and there were significant differences between two groups（ P 〈0.05）. Compared with control group, the day of mechanical ventilation, stay time in ICU and the mortality in treatment group obviously decreased（ P 〈0. 01）. Conclusion] Ulinastatin combined with continuous blood purification can increase the achievement ratio of the rescue of ARDS and reduce the mortality.
