垂体后叶素配伍米索前列醇预防产后出血156例疗效观察

目的]观察垂体后叶素配伍米索前列醇预防产后出血的临床疗效.方法]足月正常头位产妇312例,随机分为观察组和对照组,每组各156例.观察组以垂体后叶素配伍米索前列醇预防产后出血,对照组以催产素预防产后出血.观察比较产后30 min、2 h、产后24 h出血量,产后出血发生率,用药前、用药后10 min、2 h血压变化.结果]产后2 h及产后24 h观察组的出血量明显小于对照组,且差异有显著性( P <0.05).相同时间段观察组与对照组血压比较差异无统计学意义( P >0.05).产后出血率比较,观察组明显小于对照组,且差异有显著性( P <0.05).结论]对于存在高危产后出血风险的孕产妇采用垂体后叶素配伍米索前列醇联合应用可减少产后出血.

Observation on Efficacy of Vasopressin Combined with Misoprostol for Preventing Postpartum Hemorrhage

Objective]To observe the clinical efficacy of vasopressin combined with misoprostol for preventing postpartum hemorrhage. Methods] Totally 312 parturients with full-term and normal head position were randomly di- vided into observation group and control group with 156 cases in each group. Observation group received vasopressin combined with misoprostol for preventing postpartum hemorrhage, while control group received oxytocin for the same purpose. The bleeding volume at 30min, 2h and 24h after childbirth, the incidence of postpartum hemorrhage and the change of blood pressure before and at 10min and 2h after medication were observed and compared between two groups. Results]The bleeding volume at 2h and 24h after childbirth in observation group were obviously less than those in control group, and there was significant difference（ P 〈0.05）. There was no significant difference in blood pressure at the same observation period between observation group and control group（ P 〉0.05）. The incidence of postpartum hemorrhage in observation group was obviously lower than that in control group, and there was significant difference（ P 〉0.05）. Conclusion]Vasopressin combined with misoprostol for pregnant and parturient women with high risk of postpartum hemorrhage can reduce postpartum hemorrhage.
