胸锁乳突肌瓣在腮腺良性肿瘤手术中的应用

目的探讨腮腺良性肿瘤切除术同期行胸锁乳突肌瓣转移修复缺损区预防术后面部凹陷畸形及味觉出汗综合征的临床效果。方法42例腮腺良性肿瘤患者行腮腺肿瘤及部分腮腺或全叶切除术,其中18例同期行胸锁乳突肌瓣转移修复术,术后6-18个月观察面部凹陷畸形程度及味觉出汗综合征。结果常规手术组患者均有不同程度的面部凹陷畸形,味觉出汗综合征的发生率为54.2%,胸锁乳突肌瓣转移修复术组面部凹陷畸形均不明显,味觉出汗综合征的发生率为11.1%,两组间差异有统计学意义。结论胸锁乳突肌瓣转移修复术能明显改善腮腺肿瘤术后面部畸形,有效降低味觉出汗综合征发生率。

Application of sternocleidomastoid muscle flap in operation for parotid benign tumors

Objective To explore the effects of sternocleidomastoid muscle flap transfer during parotidectomy on the repair of facial deformities,and prevention of Freyp's syndrome.MethodsThe parotid tumor resection plus superficial lobectomy or parotidectomy was performed on 42 patients.Sternocleidomastoid muscle transfer was done at the same time on 18 patients.The treatment outcome was observed and a follow-up of 6-18 months was done to observe the deformity and Freyp's syndrome.ResultsAll the incisions healed well.The normal operation group patients had face deformity in different extent,and the Freyp's syndrome rate was 54.2%,and patients who did sternocleidomastoid muscle transfer showed good figuration of the parotid region.There were two patients had Freyp's syndrome(11.1%) in sternocleidomastoid muscle transfer group.There was a remarkable difference between the two operation groups.ConclusionThe sternocleidomastoid muscle flap transfer can obviously improve post-operative deformity,and prevent the incidence of Freyp's syndrome.
