老年人垂体腺瘤诊治分析(附108例报告)

目的 探讨老年人垂体腺瘤的发病特点、诊治原则和方法.方法 回顾性分析108例老年人(≥60岁)垂体腺瘤患者的临床资料,并与同期住院的其他年龄组970例垂体腺瘤患者相比较,对其发生率、临床表现、肿瘤病理类型及手术入路等进行统计学分析.结果 老年垂体腺瘤患者约占垂体腺瘤患者的10%,其特殊性在肿瘤大小、临床表现、术前合并疾病、手术方式、治愈率、术后严重并发症等方面具有统计学意义.结论 老年人垂体腺瘤起病隐袋,多见大型、侵袋性肿瘤,临床表现为视力减退明显,常合并其他系统疾病及多脏器功能减退.术前准备要求高,经颅手术较多,术后并发症多.老年人垂体腺瘤应特殊重视,综合治疗.

Analysis of management of pituitary adenoma in old age

Objective To study the pattern of illness and principle of management of pituitary adenoma in elder people. Method Clinical findings from 108 cases with pituitary adenoma in elder patients (over 60 years)were studied retrospectively, and were compared with 970 cases of different age who were hospitalized at the same period. The results were processed statistically and the incidence rate, clinical manifestations,pathological type and choices of surgical approach were figured out. Results Elder patients accounted for about 10% of all cases with pituitary adenoma. The specificities in size of the tumor, clinical manifestations, pre- operative complications, choices of operative procedure, recovery rate and postoperative complications were found statistically significant. Conclusions The onset of pituitary adenoma is insidious in elder people. Most cases show off bigger size and invasive growth pattern. Clinical features include prominent visual deterioration and patients are usually associated with pathological change of other systems and even functional failure of other organs. They require strict pre - operative preparation since the risk of operation is high and the patients are vulnerable to various types of post - operative complications. The proportion of trans - cranial approach had been increased in adopted surgical procedures. Pituitary adenoma in elder people should be paid special attention and managed comprehensively.
