不明原因发热患者146例临床分析

目的探讨不明原因发热(FUO)的病因。方法回顾性地总结分析2000年1月至2004年12月间在我院住院且符合FUO诊断标准的患者146例。结果146例患者经各种检查或特异性治疗最后明确诊断者有128例,确诊率86.9%。病因感染性疾病88例,占60.3%,其中结核病、伤寒、病毒感染最多见;血管结缔组织疾病11例,占7.5%;肿瘤性疾病13例,占8.9%;其他疾病16例,占11.0%;不明原因的18例,占12.3%。结论感染性疾病仍是FUO的主要病因,结核病尤其是肺外结核的发病率有增高趋势,结缔组织病和肿瘤性疾病在本组FUO病因中也占重要地位,大多数FUO经仔细的临床检查和分析是可以得到确诊的。

Clinical analysis on 146 cases of origin-unknown fever

Objective To investigate the cause of fever of unknown origin (FUO).Methods The clinical data of 146 patients with FUO admitted to the first clinical hospital of Shanxi medical university from January 2000 to december 2004 were analyzed.Results After various examinations and specific treatment the definite diagnosis was finally made in 128 cases (86.9%).Among them 88 cases (60.3%) were caused by infections.Eleven patients suffered from collagen vascular disease (CVD).8.9% (13/146) of the FUO cases were diagnosed as malignancy.Sixteen cases (11.0%) were due to other disease,and in 18 cases (12.3%) the cause was still unknown after hospitalization.Conclusion Infection is the main cause of FUO;the incidence of tuberculosis was the highest among the various infectious diseases as the cause of FUO;patients with extrapulmonary tuberculosis presented more frequently with FUO than those with intrapulmonary tuberculosis.CVD and malignancy were also major causes of FUO,and the incidence of malignancy increased with the lapse of time.
