| 摘 要： | 目的:探讨外伤性蛛网膜下腔出血与脑血管痉挛的关系。方法:36例患者经CT扫描或腰穿检查确诊为外伤性蛛网膜下腔出血,应用改良Fisher分级评估CT,对比经颅多普勒检查及颅内压变化。结果:出现脑血管痉挛13例,其中3例为重度血管痉挛,2例形成缺血性梗死,血管痉挛程度与出血量及部位有关。结论:外伤性蛛网膜下腔出血可引起血管痉挛,从而加重病情,应予以重视。

| Abstract: | Objective To investigate the cerebral vasospasm(CVS) and its relationship with the traumatic subarachnoid hemorrhage(tSAH). Method 36 patients suffered from tSAH diagnosed by CT scanning or lumbar puncture were observed. All the patients were examined by Transcranial Doppler(TCD) regularly. We evaluated the CT imaging by improved Fisher grade and compared the change of intracranial pressure (ICP), velocities of MCA and lindergard index during the period between 3 and 14days after head injury. Results CVS was found in 13 patients and 3 patients with severe CVS. There is significant correlation between the CVS and the amount or main site of tSAH. Conclusion CVS caused by tSAH may lead to cerebral ischemic infraction and may influence the prognosis. |
