彩色多普勒超声在乳腺癌TNM分期中的应用

目的探讨彩色多普勒超声检查对乳腺癌TNM分期的评价能力。方法108例乳腺癌患者于术前1周内均经临床乳房触诊分期和彩色多普勒超声分期,并与术后1周内病理分期(用作金标准)相比较。结果术前彩色多普勒超声检查对TNM分期的准确率与术后病理分期无明显差异(P>0.05),而显著高于术前临床乳房触诊分期(p<0.01)。该两种方法的TNM分期准确率分别为92%和76%。临床触诊对腋淋巴结的误诊率与漏诊率分别为15%和17%,而彩超仅为5%和4.6%。结论彩色多普勒超声对乳腺癌术前TNM分期具有明显优势,且能减少术前单靠临床触诊所导致的盲目性。

Application of color Doppler ultrasound in the TNM staging of breast carcinoma

Objective To investigate the capacity of color Doppler for evaluating the TNM staging of breast carcinoma. Methods Hundred-eight patients with breast carcinoma underwent TNM staging by color Doppler and clinically mammary touch within a week before the surgery and then were compared with that by pathology within a week after the surgery (taken as a gold standard).Results The accuracy of TNM staging by coloe Doppler was approximate to that by postoperative pathology(p > 0.05) but it was significantly higher than that by preoperative mammary touch(p < 0.01),the accuracy of the both methods was 92 %and 76 %,respectively.The misdiagnostic and leak-diagnostic rate of preoperative axillary lymphonodus touch was 15 %and 17 %,respectivly,while those of preoperative color Doppler ultrasound were 5 %and 4.6 %,respectively.Conclusion Color Doppler ultrasound has apparent advantage in preoperative TNM staging of breast carcinoma and can reduce the blind treatment resulting from single mammary touch.
