区域性门静脉高压症的诊断及治疗(附五例报告)

目的 探讨区域性门静脉高压症的诊断及治疗方法.方法 回顾性分析我院1992年～2002年诊治的区域性门静脉高压症5例的临床资料.本组5例均行手术治疗.结果 本组无并发症发生,无手术死亡病例.术后随访5年,2例肿瘤病人在1年内死亡,其余3例未发生消化道出血.结论 对于未发现肝脏疾病而有胃底静脉曲张、脾功能亢进的病人,应考虑本病.本病可经脾切除加贲门周围血管离断术治愈.

Treatment and diagnosis of regional portal hypertension:A report of 5 cases

Objective To study the diagnosis and treatment of regional portal hypertension.MethodsThe clinical data of 5 patients with regional portal hypertension treated in Xinghua Hospital from 1992 to 2002 were retrospectively analyzed.ResultsAll of the patients in this group underwent surgical operation.No postoperative complications occurred.There was no operative mortality.During a follow-up period of 5 years,2 patients died from cancer of the pancreas in one year,and the remaining 3 cases had no massive digestive tract re-bleeding.ConclusionRegional portal hypertension is a kind of prehepatic portal hypertension.When patients have gastric varices or hypersplenism in the absence of liver disease,regional portal hypertension should be considered.The extensive esophagogastric devascularization with splenectomy is recommended.
