| 摘 要： | 目的研究不同结核病细菌学检查方法对菌阳肺结核患者的检出水平,探索最佳的结核病实验室检查方法。方法采用痰抗酸杆菌直接涂片检查和痰结核菌分离培养的方法检测菌阳肺结核患者,统计分析不同咳痰时间和不同性状痰标本的涂片检查的阳性检出水平;比较三涂一培、三涂两培、三涂三培的不同实验方法组合的菌阳肺结核患者的检出率。结果1911份初诊即时、夜间、清晨咳出的干酪痰、血痰、黏液痰、唾液标本的涂片、涂片+培养的检出率各有显著性差异。清晨痰、干酪痰检出率最高为29.82%、65.24%(χ2=62.428,P<0.01;χ2=323.650,P<0.01);三涂两培、三涂三培的菌阳检出率为52.59%、52.90%,三涂三培与三涂两培的菌阳检出率无差异(χ2=0.013,P>0.05)。结论清晨干酪痰阳性检出率最高,采用三涂二培的方法可最大限度的发现菌阳肺结核患者。
