经侧裂-岛叶入路显微手术治疗高血压脑出血效果

目的观察经侧裂-岛叶入路显微手术治疗高血压脑出血的疗效,探讨术中、术后保护的策略。方法对26例经侧裂-岛叶入路治疗高血压脑出血病人的临床资料进行回顾性分析。结果术后3d病人GLASGOW评分13～15分8例,9～12分14例,6～8分3例,3～5分1例,病人瞳孔均回缩。术后1周病人偏瘫侧肢体肌力改善。26例病人出院后随访半年,日常生活活动评分评定预后：Ⅰ级2例,Ⅱ级12例,Ⅲ级6例,Ⅳ级4例,Ⅴ级2例。结论经侧裂-岛叶入路治疗高血压脑出血疗效较好,预后好。

TRANSSYLVIAN-INSULAR APPROACH MICROSURGERY FOR HYPERTENSIVE CEREBRAL HEMORRHAGE

Objective To investigate therapeutic efficacy of transsylvian-insular approach（TSIA） microsurgery for hypertensive cerebral hemorrhage（HCH）,and discuss the protection before and after surgery.Methods A retrospective analysis was done in 26 cases who underwent TSIA microsurgery for HCH.Results Three days after surgery,GLASGOW score showed13-15 marks in eight cases;9-12 marks in 14;6-8 in three;and 3-5 in one.All the pupils were retracted.One week after surgery,the muscle strength of hemiplegic limbs improved.All the patients were followed for six months after discharge fro hospital,the score of activities of daily living was made to predict their diagnosis as follows： grade Ⅰ,two cases;grade Ⅱ,12 cases;grade Ⅲ,six cases;grade Ⅳ,four cases;and grade V,two cases.Conclusion The outcome of hypertensive cerebral hemorrhage treated with transylvian-insular approach microsurgery is satisfied in terms of efficacy and prognosis.
