促进外伤性截瘫病人顺利排尿方法的探讨

[目的]探讨促进外伤性截瘫病人早日脱离尿管,减少尿路感染的方法。[方法]将66例外伤性完全性截瘫病人随机分为两组:康复组在脊髓休克期留置尿管3d后给予夹管,定时按摩按压膀胱排尿和病人有意识排尿活动训练;膀胱功能强化训练1周~2周后,在膀胱充盈时采用辅助按摩按压等方法下拔除尿管;并通过多饮水保持每天尿量在2500mL以上,进行生理性膀胱冲洗。对照组按护理常规护理。[结果]康复组尿管留置时间明显缩短,尿路感染率明显降低。[结论]早期膀胱康复训练,生理性膀胱冲洗和可行的拔管方法能促进外伤性截瘫病人顺利排尿。

Probe into methods of promoting miction smoothly for traumatic paraplegia patients

Objective: To probe into the methods of promoting extubating of urinary catheter as early as possible for traumatic paraplegia patients and reducing their urinary tract infection. Methods: A total of 66 cases with traumatic completely paraplegia were divided into rehabilitation and control group randomly. Patients of rehabilitation group were clamped indwelling urinary canal after three-day's spinal shock period, and urinary bladder was massaged timely for urination and consciousness of urination training. After one to two weeks of intensified bladder function training, urinary catheter was pulled out based on filling and massaging of bladder. These patients were asked to drink much water and kept daily urine volume more than 2 500 mL physiological douching of bladder. Patients of control group were just given nursing care as in routine. Results: The time of indwelling urinary catheter in rehabilitation group patients was apparently shortened, and the incidence of urinary tract infection was remarkably decreased. The urination manner of them was also improved. Conclusion: Early bladder rehabilitation training, physiological douching of bladder and feasible extubation could promote miction smoothly for traumatic paraplegia patients.
