子宫颈LLETZ手术治疗CINⅢ的临床疗效分析

摘    要:目的评价子宫颈移行区大环切除(LLETZ)手术治疗子宫颈上皮内瘤变(CIN)的临床疗效。方法回顾性分析我院2003年5月~2008年2月间阴道镜下多点活检诊断CINⅢ患者558例LLETZ手术治疗后的随访结果和临床疗效。结果CINⅢ组患者的平均年龄(34.72±7.27)岁,年龄≤35岁患者所占比例为61.8%。LLETZ手术后切缘CINⅡ/Ⅲ病灶残留率为4.8%,其中CINⅢ残留率为2.0%。LLETZ手术术前术后诊断符合率是72.2%,其中CINⅢ患者术后诊断Ⅰal期子宫颈癌58例,占10.4%。随访12~48个月,目前未见复发。结论LLETZ手术可用于诊断和治疗CINⅢ,但应严密随访,尤其是术后有残留患者。

Abstract:Objective To evaluate the effect of large loop excision of the transformation zone (LLETZ) on cervical intraepithelial neoplasia (CIN) Ⅲ. Methods Retrospective analysis on the clinical effect and follow - up results of 558 women with CIN Ⅲ. who were treated with LLETZ in our hospital from May 2003 to Feburay 2008, was carried out. Results The mean age of patients with CINⅢ was (34.72±7.27) years. The incidences of residual diseases on cutting edge of CIN Ⅱ / Ⅲ group after LLETZ were 4.8 %. The coincidence of pre - and postoperative diagnosis in CIN m group were 72.2%. Fifty- eight (10.4%) patients in CINⅢ group became cervical cancer stage Ial after LLETZ. The patients were followed up for 12-48 m, and no recurrence was found. Conclusions LLETZ can be used in the treatment of CIN Ⅲ.
