泌尿系结石的多层螺旋CT诊断

摘    要:目的 探讨多层螺旋CT对泌尿系结石的诊断价值.方法 对65例临床及其它检查怀疑泌尿系结石的患者先进行CT平扫,然后经肘前静脉注射对比剂后延迟6-15min行分泌期扫描,扫描所得原始数据传至ADW4.2 工作站进行泌尿系成像.其中男43例,女22例,年龄16-70岁,平均48.5岁.结果 65例患者中,CT发现单纯肾结石35例,单纯输尿管结石8例,单纯膀胱结石4例,肾结石合并输尿管结石11例,肾结石、输尿管结石及膀胱结石共存者5例,2例未发现结石.所有结石在CT图像上均表现为高密度影,直径2mm-5cm不等,9例伴有继发尿路梗阻征象.与CT横轴位图像相比,泌尿系成像显示结石大小、形状、位置更为形象、直观.结论 多层螺旋CT是诊断泌尿系结石的有效方法 .泌尿系成像诊断结石更为形象、直观.

Abstract:Objective To explore the diagnostic value of multi-slice spiral CT in urinary calculi. Methods 65 patients (included 43 males and 22 females, aged from 16 to 70 years old, mean age of 48.5 years old) suspected of urinary stones by clinic and other studies underwent multi-slice spiral CT, plain scanning first, then excretory stage scanning 6-15 minutes after the start of contrast agents through elbow veins, then raw data were transferred to ADW4.2 workstation for reconstructions of urography including maximum intensity projection (MIP), volume rendering(VR) and virtual endoscopy(VE). Results Among 65 patients, 35 cases of patients with renal stones, 8 patients with ureter stones, 4 patients with bladder stones, 11 patients with renal stones and ureter stones both, and 5 patients with renal, ureter and bladder stones were detected by multi-slice spiral CT respectively. All stones manifested high density in CT. The size of stones ranged from 2mm to 5cm in diameter, and 9 patients combined with urenary obstruction. Compared with axial view, the size, shape and location of stones expressed well in CT urography. Conclusion Multi-slice spiral CT is the optimal modality to diagnose urenary stones, and CT urography detect stones more activatedly and intuitively.
