血浆D一二聚体、cTnI联合检测在急性肺栓塞中的诊断价值

【摘要】目的探讨血浆D一二聚体（D-D），肌钙蛋白I（cTnI）联合检测在急性肺栓塞中的诊断价值。方法D一二聚体用STAGOSTACompact全自动血凝仪检测，cTnI用化学发光免疫法检测，仪器为罗氏电化学发光免疫分析仪，分别检测35例急性肺栓塞（APE）和40侧正常健康人血浆中的D一二聚体和cTnl水平，所有数据用SPSS11．5软件包进行分析。结果急性肺栓塞患者血浆D一二聚体和cTnl水平均显著高于正常组，差异有统计学意义（Ko．05）。D一二聚体和cTnI单独检测APE的灵敏废分别为83．1、82．3％，特异度分别为36．7％、35．2％，两者联合检测APE的灵敏度为98％、特异度为58．3％。结论血浆D一二聚体．cTnl在APE诊断中具有重要临床意义，联合检测血浆中D一二聚体、cTnI的水平有利于提高急性肺栓塞的诊断率。

Study on the Value of Combined Detection of D-D,CTnI in APE

Objective To evaluate the value of combined analysis of D-D,cTnI in the APE.Methods Plasma level of D-D in each patient was determined with STAGOSTA Compact,and Plasma levels of cTnI was determined in chemoluminescence method.plasma levels of D-D and cTnI were collect from 35 cases with APE,40 cases helthy volunteers.Statistical analysis was performed by using SPSS 11.5.Results The concentration of D-D and cTnI in APE group were higher than those helthy volunteers group (P<0.05).The sensitivity rates of D-D and cTnI in APE were 83.1,82.3,respectively,the specific rates were 36.7,35.2,respectively.In combined assay of D-D and cTnI,the sensitivity rate was 98,the specificity rate was 58.3.comparing with the single assay,combined analysis had much higher diagnostic sensitivity.Conclusion The assay of D-D and cTnI is significant in diagnosis of APE.In Combined assay of D-D and cTnI can improve the rate of early APE.
