乳腺癌前哨淋巴结微转移的检测及其临床意义

目的探讨对乳腺癌前哨淋巴结（sentinel lymph node,SLN）微转移更为有效的检测方法及临床意义。方法我们对50例乳腺癌患者进行了前哨淋巴结活检,进行常规的病理切片检查和SLN中CK-19的RT-PCR检查,然后运用统计软件SPSS10.0对数据统计分析。结果 50例乳腺癌患者成功进行了前哨淋巴结活检,共检出了SLN 96枚。常规的病理切片检查和RT-PCR检查两者的敏感性、准确性和假阴性率分别为29.4%（10/34）,56%（28/50）,64.7%（22/34）,94.1%（32/34）,96%（48/50）,5.9%（2/34）。结论 SLN中CK-19的RT-PCR检查可以提高SLN微转移的检出率,而且可以减少假阴性率,值得在临床推广使用。

Detection and its clinical significance of micrometastasis of sentinel lymph nodes in breast cancer

[ Objective ] To investigate a more rational detection method for micrometastasis of sentinel lymph node（SLN） and the clinical significance of detecting the SLN micrometastasis in breast cancers. [ Methods ] 50 breast cancer patientsunderwent sentinel lymph node biopsy.The intraoperatively resected SLNs were sent for pathological examination and the RT-PCR of SLN were detected of CK19.SPSS10.0 was used for statistical analysis. [ Resluts ] Sentinel lymph node biopsywas successful for the 50 breast cancer patients.A total of 96 SLNs were detected staining.The sensitivity, coincidence rate, specificity and false negative rate for pathological examination and RT-PCR were 29.4% （10/34）, 56% （28/50）, 64.7%（22/34）, 94.1% （32/34）、96 % （48/50） and 5.9 % （2/34）, respectively. [ Conclusion ] The RT-PCR can increase the detection rate of SLN micrometastases and reduce the false negative rate.And we recommend this method in clinical application.
