胎儿脐带绕颈433例临床分析

目的 探讨胎儿脐带绕颈的产前检查，产程特点及分娩方式。方法 对在临沂人民医院１９９８年７月至１９９８年１２月分娩的胎儿脐带绕颈的病历资料进行回顾性分析。结果 胎儿脐带绕颈占同期分娩胎儿总数的２１．３％，产前Ｂ超检出答合率为９３．１％，脐带绕绒周数检出符合率为６４．２％，电子胎心监护变异减速又占７．４％，产程中胎儿宫内窘迫占２０．６％，且与绕颈周数有关。剖宫产分娩者４８．７％，自然分娩者占４５．０％

CLINICAL ANALYSIS OF CORD AROUND NECK IN 433 FETUSES

Objective To explore the prenatal examination ,birth process and delivery ways of the fetuses with cord around neck.Methods Retrospective study was made among the neonates with cord around neck born in Linyi City People's Hospital from July to December in 1998.Results Fetuses with cord around neck accounted for 21.3% of the homochronous ones.The coindience rate of prenatal examination through ultrasonic imaging was 93.1%.The coincidence rate of cord circle detection was 64.2%.Electronic heart monitoring showed that only 7.4% of the neonates had variant deceleration and 20.6% of them suffered from intrauterine distress related to the circles of cord around neck.Of the neonates with cord around neck,48.7% were born through cesarean section,45% natural delivery and 6.2% transvaginal aids.The suffocating rates in the above three group were 5.2%,7.4%,and 4.6% respectively.No significant differences were found among the three groups.Conclusion Cord around neck is a kind of common abnormality of umbilical cord.High detectable rate,close observation in birth process and immediate handling of intrauterine distress can greatly reduce the incidence of cesarean section and serious complications in mother and infant.
