| 摘 要： | 目的结合病理结果分析桥本氏甲状腺炎合并乳头状癌的超声图像特征,以提高对其诊断的准确率。方法对130例桥本氏甲状腺炎合并结节患者术前进行超声诊断,结合相应的术后病理结果,分析比较桥本氏甲状腺炎合并乳头状癌与桥本氏甲状腺炎合并良性结节超声图像特性的差别。结果桥本氏甲状腺炎合并乳头状癌患者的结节在超声图像上有如下特征:结节形状不规则,边界欠清晰;结节内部呈低回声且内部回声不均匀;结节中多见钙化灶。与单纯桥本氏甲状腺炎结节超声表现有明显差异。结论单纯桥本氏甲状腺炎合并乳头状癌在超声声像图上存在一些特性,如发现桥本氏病合并低回声结节、结节内部存在钙化时,应高度怀疑恶性结节的存在,必要时可行超声引导下细针穿刺以取得术前病理诊断结果。
