玻璃体切除联合药物治疗内源性眼内炎

目的评价玻璃体切除手术(PPV)与抗生素联合使用对内源性眼内炎的治疗效果。方法回顾分析了2002年～2005年本院内源性眼内炎12例的玻璃体切除手术,以及术前和术后抗生素使用情况。结果13例中,发病前有高热者9人(75%),病原确认或培养阳性为4例(33%),全部病人接受了玻璃体切除手术联合抗生素治疗,5例给予玻璃体腔注药。手术后维持手动以上视力的有6人(50%),2例最终眼球摘出(17%)。结论发热病人出现眼部症状时,应高度警惕出现眼内炎的可能,尤其是有糖尿病或接受创伤性操作的病人。玻璃体切除手术联合抗生素治疗,有助于保存病人的有用视力。

Outcome of vitrectomy combined with antibiotic chemotherapy on endogenous endophthalmitis

Objective To investigate role of pars plana vitrectormy (PPV) and antibiotic chemo- therapy in the treatment of 12 eyes of endogenous endophthalmitis. Methods Records of 12 consecutive patients diagnosis with endogenous endophthalmitis were retrospectively reviewed and the outcome of treat- ment were evaluated. Results The patient's ages ranged from 2-83 years, nine (75%) of all cases had high fever, only four (33%) of the 12 were culture positive, six (50%) eyes obtained FC or better acuity vision. Two eyes (17%) were enucleated finally. Conclusions Various predisposing conditions could attribute the develop- ment of endogenous endophthalmitis, including TB, diabetic mellitus, et al. physicians should be alert to pa- tients of fever. PPV in conjunction with intravenous antibiotics plays an important role on preserve useful vision of patients suffered, antibiotics should be continued for at least 2-3 weeks or longer.
