几种肿瘤标志物对消化道肿瘤的诊断价值探讨

目的:了解肿瘤标志物CA50、CA199、CA242、CA724在消化道肿瘤中的表达情况,为临床诊断提供参考依据。方法:选择2005年1月～2007年6月期间在我院就诊的消化道系统肿瘤患者、消化道良性疾病患者、同期健康体检人群为研究对象,检测所有对象的CA50、CA199、CA242、CA724水平。结果:(1)恶性肿瘤组各指标均高于良性疾病组及健康对照组(P<0.05),但各指标在良性疾病组及健康对照组之间差异无统计学意义。对比不同部位肿瘤的结果表明,CA50及CA199均以胰腺癌高于胃癌和结肠癌(P<0.05),而胃癌及结肠癌之间差异无统计学意义;CA242及CA724在不同肿瘤患者间的水平差异无统计学意义。(2)分别以CA50、CA199、CA242、CA742的正常值范围为诊断标准时,对每种肿瘤的诊断均具有一定的误诊率及漏诊率。结论:CA50、CA199、CA242、CA724在消化道肿瘤的诊断中具有重要意义,但进一步确诊尚需联合其他辅助诊断手段。

A Study on the Diagnosis Value of Several Tumor Markers on Tumor of Digestive System

Objectives：To explore the expression level of CA50、CA199、CA242、CA724 in tumor of digestive system, so that to provide information for clinical diagnosis. Methods： The patients with digestive system tumor, with benign disease, and healthy people were selected as the study objects. The level of CA50、CA199、CA242、CA724 of everyone were tested. Results： （1） The level of the four parameters of the malignant tumor group was higher than those of the benign group and healthy control group （P〈0.05）. However, there Was no statistical difference of the four parameters between the benign group and healthy control group. When the level of different part tumor was analyzed, we found that the level of CA50 and CA199 pancreas tumor group were higher than that of the gastric carcinoma group and colon cancer group. But, there was no statistical difference of the level of CA242 and CA724 among the three groups. （2） If each parameter was selected as the diagnosis standard, there existed misdiagnosis and missed diagnosis.Conclusions：CA50、CA199、 CA242、CA724 play a very important role in the diagnosis of digestive system cancer. However, only the tumor markers and other diagnosis measures were concerned together, the right diagnosis can be elicited.
