盐酸羟考酮控释片治疗晚期癌症疼痛的临床疗效及安全性分析

目的:观察盐酸羟考酮控释片治疗晚期癌症中重度癌性疼痛的疗效及不良反应。方法:回顾性分析我院肿瘤科2011年2月-2014年2月间,216例晚期中重度癌性疼痛患者予以盐酸羟考酮控释片镇痛治疗情况,初始剂量10~20 mg,q12h,根据疼痛情况调整剂量至无痛或基本无痛状态,评估镇痛效果和不良反应。结果:经最终剂量滴定后,盐酸羟考酮控释片用药剂量范围10~130 mg,q12h。疼痛完全缓解率52.8%(114例),部分缓解43.1%(93例),总的癌痛缓解率(CR+PR/总人数)95.8%。不良反应主要有便秘(84例)、头晕(33例)、恶心呕吐(45例)、厌食(27例)、嗜睡(9例)、尿储留(3例),但症状轻微且可控。结论:盐酸羟考酮控释片是晚期癌症中重度疼痛患者的有效治疗选择,其可以通过剂量滴定个体化给药的方式显著改善癌症患者的生活质量,且耐受性良好。

Analysis of clinical efficacy and safety of oxycodone hydrochloride prolonged-release tablets against terminal cancer pain

OBJECTIVE To observe the efficacy and safety of oxycodone hydrochloride prolonged-release tablets in treatment of terminal cancer with moderate or severe pain. METHODS From February 2011 to February 2014, a total of 216 patients in oncology department of Fujian Union Hospital were treated with oxycodone hydrochloride prolonged-release tablets at an initial dose of 10-20 mg/q12h. Dose of oxycodone hydrochloride prolonged-release tablet was adjusted until pain disappeared or controlled, its efficacy and side effects were observed. RESULTS The final titrated dose of oxycodone hydrochloride prolonged-release tablet was in the range of 10-130 mg/q12 h. Total pain relief rate was 95.8%, among which the rates of complete response and partial response were 52.8% and 43.1%, respectively. The main side effects included constipation (84), dizziness (33), nausea (45), vomiting (27), anorexia (9) and drowsiness (3). CONCLUSION Oxycodone hydrochloride prolonged-release tablet is one of the effective drugs in controlling terminal cancer with moderate and severe pain, and can improve the living quality for patients.
