苯巴比妥血药浓度监测与临床疗效的相关性分析

目的：为临床癫痫患者苯巴比妥的合理用药提供参考。方法：采用荧光偏振免疫法,对369例癫痫患者服用苯巴比妥进行血药浓度监测,并对结果及疗效进行分析总结。结果：208例次苯巴比妥血药浓度监测结果在有效浓度范围内,占总例次的56.37%,而有16例次血药浓度在正常范围,未能控制癫痫症状。血药浓度高于或低于正常浓度有161例次,占43.63%。结论：苯巴比妥血药浓度监测对合理用药有重要的指导作用,血药浓度个体差异大,临床应用时需个体化用药。

Study on the Relationship Between Serum Concentration of Phenobarbital and Therapeutic Efficacy in Pediatric Patients with Epilepsy

Objective To study the relationship between serum concentration of phenobarbital and therapeutic efficacy in children with epilepsy.Methods Serum concentrations of phenobarbital（morning specimen） were measured with fluroscence polarization immuno-assay（FPIA） in 369 pediatric patients with epilepsy on regular conventional dose of phenobarbital.Results Of the 369 patients,serum concentrations of the drug were within the therapeutic range（1540μg/ml） in 208 patients（56.37%） with 16 cases of therapeutic failure.However,among the 161 patients with higher（29） or lower（132） than conventional serum phenobarbital therapeutic range,108 were still considered treatment satisfactory.Conclusion Therapeutically effective serum concentration of phenobarbital is highly individulized,monitoring of which would be usefal for treatment guide.
