急性脑梗死后脉压的相关性研究

摘    要:目的通过对急性脑梗死患者的人院脉压分析,研究脉压增大与脑梗死的发生、严重程度的相关性。方法选住院治疗的急性脑梗死患者300例,和同期来我院的体检人员199例为对照组,记录血压各个参数(收缩压、舒张压、脉压)、脑梗死组的神经功能缺损评分,并进行统计分析。结果(1)病例组与对照组的脉压有显著性差异,病例组脉压高于对照组。脉压与收缩压的相关性更为密切。(2)在缺血性脑梗死组,脉压最大的分布区域在60~69mmHg,占27.7%;对照组脉压最大的分布区域在40~49mmHg,占35.7%。显示脑梗死时脉压大于60mmHg的发生率较高。(3)随着年龄的增加,脉压增大。两组的同年龄分组比较,40~69岁脑梗死组的脉压水平明显高于对照组。(4)脉压的增大与神经功能缺损评分不相关。结论脉压的增大与脑梗死的发生有关,是脑梗死发生的重要评价因素,但脉压的变化并不与脑梗死的严重程度相关。

Abstract:Objective To study the relationship between increased pulse pressure and the occurrence of acute cerebral infarction and the predictive role of pulse pressure by analyzing the data of pulse pressure recorded just as the patients were hospitalized. Methods The pulse pressures of 300 inpatients with cerebral apoplexy and 199 persons receiving health examinations in the same period (control group) were examined correlation analysis between the systolic pressure,diastolic pressure,pulse pressure and neurologic impairment evaluation was done. Results (1)The pulse pressure of case group was significantly higher than that of control group and closer relationship was shown between pulse pressure and systolic pressure. (2)In case group,27.7% patients had maximum pulse pressures ranging from 60 to 69mmHg in contrast with that 35.7% patients in control group got maximum pulse pressures distributing between 40 and 49mmHg,which indicated that the incidence of cerebral apoplexy was higher when the patient's pulse pressure was higher than 60mmHg. (3)The pulse pressure increased with the growth of the patient's age. (4)The increasing of pulse pressure had nothing to do with neurologic impairment evaluation. Conclusion The increasing of pulse pressure is relevant to the occurrence of cerebral apoplexy and is also an important evaluating factor,but it is not relevant to the severity of cerebral apoplexy.
