后循环脑梗死治疗方法的临床研究

目的探讨急性后循环脑梗死患者抗凝、静脉溶栓及动静脉联合溶栓治疗的效果。方法回顾性分析60例后循环脑梗死患者资料,发病时间为12～72 h。根据治疗方法不同,将60例患者分为抗凝治疗组(30例)、静脉溶栓组(20例)和动静脉联合溶栓治疗组(联合治疗组,10例),采用美国国立卫生研究院卒中量表(NIHSS)评定患者临床症状改善情况,评价治疗24 h和治疗7 d时的效果。结果 3种方法治疗24 h和治疗7 d的疗效比较,静脉溶栓组的治疗效果优于抗凝治疗组,联合治疗组的治疗效果优于抗凝治疗组和静脉溶栓组,组间比较差异均有统计学意义(P<0.05)。结论后循环脑梗死患者病情危重,可以根据个体情况,采取各种联合治疗,溶栓时间窗可以延长。

Clinical Study on the Therapeutic Approach in Posterior Circulation Cerebral Infarction

Objective To investigate the therapeutic effects of anticoagulation,intravenous thrombolysis,combined intra-arterial and intravenous thrombolysis in acute posterior circulation cerebral infarction(PCCI).Methods A retrospective analysis was conducted on 60 PCCI patients with onset time between 12 and 72 hours.According to their therapeutic approach,the patients were divided into three groups: anticoagulation group(30 patients),intravenous thrombolysis group(IT group,20 patients),combined intra-arterial and intravenous thrombolysis group(CIIT group,10 patients).National Institutes of Health Stroke Scale(NIHSS) was used to observe the clinical symptom improvement so as to evaluate the therapeutic effects at 24 hours and 7 days after treatment.Results Both at 24 hours and 7 days of treatment,the therapeutic effect in IT group was significantly better than that in anticoagulation group,and the therapeutic effect in CIIT group was significantly better than the other two groups(P<0.05).Conclusion PCCI was a critical illness,and combined therapeutic approach can be adopted to extend the timeframe for thrombolysis.
