后腹腔镜在肾癌根治术中的应用

目的探讨后腹腔镜肾癌根治术的疗效。方法2005年2月至2008年12月对27例肾细胞癌患者行后腹腔镜根治性。肾切除术，使用免水囊法建立操作空间，用Hem—o—lock结扎夹处理肾动静脉。结果全组手术时间100～220min，平均149min；出血量30～100mL，平均55mL；术中、术后未发生严重并发症。随访2～30个月，1例发生肺转移。结论后腹腔镜肾癌根治术具有创伤小、术后恢复快等优点。用免水囊扩张方法建立腹膜后操作通道安全可行，疗效可靠。用Hem—o—lock结扎夹处理肾动静脉安全可靠，经济实用。

The application of retroperitoneoscopic in radical nephrectomy for renal cancer

Objective To explore the therapeutic effect of retroperitoneoscopic radical nephrcetomy. Methods Between Feb. 2005 and Dec. 2008, retroperitoneoscopic radical nephrectomy was performed in 27 renal cancer patients;retroperitoneal approach was structured without water balloon, and Hem-o-lock clips were used to control renal vessels during operation. Results In this study, operating time was from 100 to 220min （mean： 149min） , blood loss was from 30 to 100mL（ mean： 55mL） , and no complications occurred. During the follow-up of 2-30 months, 1 patient had lung metastasis. Conclusions Retroperitoneoscopic radical nephrectomy has the advantages of mini-invasion and rapid recovery. The method of structuring retroperitoneal approach without water balloon is safe and effective. Hem-o-lock clips is a reliable and economical device for renal vascular control.
