经支气管镜行支气管冲洗及局部注药治疗急性肺脓肿的临床研究

目的探讨经支气管镜行支气管冲洗及局部注药联合内科常规疗法治疗急性肺脓肿的疗效。方法对86例急性肺脓肿患者随机分为两组，治疗组46例予以经支气管镜行支气管冲洗及局部注药联合内科常规治疗；对照组40例予以单纯内科常规治疗。结果治疗组治疗成功45例，失败1例；对照组治疗成功32例，失败8例，两组治疗成功率比较差异有统计学意义（P〈0．05）。治疗组在体温恢复正常、咳痰咳嗽消失以及住院时间方面较对照组明显缩短（P〈0．01）。结论经支气管镜行支气管冲洗及局部注药联合全身用药治疗急性肺脓肿，能显著提高疗效，减少住院天数，有临床实用价值。

Clinical research of the broncholavage and local drug injection by bronchoscopy in the treatment of acute pulmonary abscess

Objective To investigate the efficacy of the broncholavage and local drug injection by bronchoscopy combined with traditional internal therapy in the treatment of acute pulmonary abscess. Methods Eighty-six patients with acute pulmonary abscess were allocated to two groups （the therapeutic group and the control group）. 46 patients of the therapeutic group received the broncholavage and local drug injection by bronchoscopy combined with traditional internal therapy treatment. 40 patients of the control group were given only traditional internal treatment. Resuits In the therapeutic group：treatment success,45 patients;failure,1 patient;in the control group： treatment success ,32 patients ;failure ,8 patients. There was significantly statistical difference in healing rate between two groups（ P 〈 0. 05 ）. Temperature recovery, disappearance of cough and expectoration and the hospitalized time in the therapeutic group shortened more obviously than those in the control group （ all the P 〈 0.01 ）. Conclusion The broncholavage and local drug injection by bronchoscopy combined with traditional internal therapy in the treatment of acute pulmonary abscess can improve the efficacy and reduce the hospitalized time and be of value in clinical recommendation.
