肺表面活性物质对新生儿呼吸窘迫综合征的临床疗效研究

摘    要:目的探讨肺表面活性物质(PS)对新生儿呼吸窘迫综合征(NRDS)的临床疗效。方法将39例NRDS患儿分两组,治疗组19例给予PS联合机械通气治疗,对照组20例单纯机械通气治疗,其余治疗两组相同。观察应用PS前后血气分析、呼吸机参数变化及比较两组患儿机械通气时间、并发症、住院时间、病死率。结果应用PS后动脉氧分压(PaO2)升高(P〈0.01),二氧化碳分压(PaCO2)、吸气峰压(PIP)、平均气道压(PEEP)及吸入氧浓度(FiO2)下降(P〈0.01);两组机械通气时间、住院天数、并发症等差异均有统计学意义(P〈0.05),病死率差异无统计学意义(P〉0.05)。结论PS对NRDS有明显疗效,与单纯机械通气比较,能改善患儿的氧合,减少机械通气时间、并发症和住院天数。

Pulmonary surfactant in treatment of neonatal respiratory distress syndrome

Abstract:Objective To explore the effect of pulmonary surfactant(PS)on neonatal respiratory distress syndrome(NRDS).Methods Nineting patients with NRDS were treated with mechanical ventilation and PS(treatment group),another 20 patients were treated with mechanical ventilation(control group).Gestational age and newborn weight were similar(P〉0.05)between two groups.All patients were evaluated in blood-gas analysis,the parameter of mechanical ventilation,mechanical ventilation time,complications,time in hospital and fatality rate.Results After PS treatment,the level of PaO2 increased remarkably(P〈0.01),and the level of PaCO2 decreased greatly(P〈0.01).The PiP,MAP and FiO2 dropped obviously(P〈0.01).The mechanical ventilation time,complications and time in hospital were different significantly between two groups(P〈0.05).The fatality rate was not different between two groups(P 〉0.05).Conclusion PS therapy has more curative effect on NRDS,it can improve oxygenation of NRDS and reduce the mechanical ventilation time,complications and time in hospital,but it can not reduce the fatality rate.
