氯吡格雷对UAP患者血清MMP-2、MMP-3、MMP-9水平的影响及疗效观察

摘    要:目的:探讨氯吡格雷对不稳定型心绞痛(UAP)患者血清基质金属蛋白酶(MMP)-2、3、9水平的影响及疗效观察。方法:70例UAP患者,随机分为观察组与对照组。两组患者均予以抗血小板聚集、扩血管、降血脂和改善心肌微循环等治疗。观察组在此基础上加用氯吡格雷75mg,1次/d,连用4周。结果:治疗4周后,两组血清MMP-2、3、9水平较前均有明显下降(P〈0.05或P〈0.01),且观察组下降值较对照组更明显(P〈0.05);同时观察组的临床总有效率明显高于对照组盯=5.29,P〈0.05)。结论:氯吡格雷治疗UAP具有较好的效果,作用与其能降低血清MMP-2、3、9水平,增强动脉硬化斑块的稳定性有关。

Abstract:Objective To investigate the effect of clopidogrel on serum metalloproteinase(MMP) -2,3,9 levels in patients with unstable angina and its clinical efficacy. Methods 70 cases of patients with unstable angina were randomized divided into observed group and control group. All patients were given conventional treatment such as anti-platelet aggregation vasodilators, expanding blood vessel, lowering blood lipid, improving myocardial microcirculation. The observed group were given clopidogre175mg, once daily for 4 weeks on this basis. Results Serum MMP-2,3,9 levels significantly decreased after 4 weeks of treatment (P 〈 0.05 or P 〈 0.01 ), and the decline in value in observed group proved more higher than that of control group ( P 〈 0.05 ), the total efficiency in the observed group presented significantly higher than those in control group ( Х^2 = 5.29, P 〈 0.05). Conclusion Clopidogrel in the therapy of unstable angina provides better clinical efficacy, and its mechanism of action may be related to significantly lowering of serum MMP-2, 3, 9 levels, and enhancing the stability of the plaque.
